Reservation Requirements
- Enroll with Oneida County Office for Aging before scheduling with 50 Forward Mohawk Valley
- Request transportation 2-3 weeks prior to the date of medical appointment
- Over 60 years of age and not receiving Medicaid benefits
- Must be ambulatory and have the mental, visual and auditory capacity to function independently without assistance from the volunteer driver
- Transportation is only provided for medical appointments (primary care physician, dental, vision, etc.) and not for ongoing medical treatment (physical therapy, radiation treatments, chemotherapy, dialysis, or procedures involving anesthesia)
- Transportation is contingent upon volunteers being available to drive
